American Family Care and the Western Connecticut Medical Group (WCMG) are joining forces to improve their referral services at numerous locations across the state.
WCMG, an affiliate of the three-hospital Western Connecticut Health Network — which recently agreed to a merger with Health Quest Systems — said patients from AFC’s nine Fairfield County locations will have improved access to WCMG if follow-up care is required.
In addition, WCMG patients who need non-emergency treatment during nights, weekends or holidays will be informed about the immediate services available at AFC’s urgent care clinics. AFC clinics would then return follow-up information to the patient’s WCMG physician to coordinate care.
“This relationship is completely aligned with our patient-centric approach to health care”, Tom Kelly, AFC’s local area executive, said.
WCMG will maintain its two existing offices that offer urgent care and walk-in services, said Richard Gemming, executive director of WCMG. Gemming said its new partnership with AFC will further its urgent care reach in retail centers.
