Visiting Nurse & Health Services of Connecticut, a home health and hospice care agency serving 25 communities in eastern and north central Connecticut, has received a $100,000 grant from the SBM Charitable Foundation to further expand its Telehealth program — a technology used for patients at risk of hospitalization due to chronic illness.
