Synergy Advanced Healthcare LLC, located at 6 Bird St., in Torrington, filed the report with the U.S. Department of Health and Human Services’ Office for Civil Rights on Nov. 4. The report is posted on the office’s data breach portal.
The listing states that electronic medical records were exposed to “unauthorized access/disclosure.” No other information is provided to indicate when the breach actually occurred, how long it took for the practice to notify authorities and affected patients of the breach, and what specific information was compromised.
On its website, Synergy Advanced Healthcare describes itself as a “veteran-owned and operated, primary care, medication management office” that is owned by Brandy R. Stepney, an advanced practice nurse and a nurse practitioner, and Keith Stepney.
The Synergy website does not include a notice of the data breach.
Officials with Synergy did not immediately respond to requests for comment.
