Deb Smith has been hired as the Live Well program coordinator for Connecticut Community Care Inc. in Bristol.
The nonprofit was selected by the Department of Social Services, Aging Services Division, to provide enhanced coordination for the Live Well program in the north central and eastern regions of the state. Live Well is the evidence-based chronic disease self-management program developed by Stanford University and is in 45 states including Puerto Rico and the District of Columbia.
Smith worked as an access coordinator in the Connecticut Community Care’s North Central Regional Office from 1999 to 2005 and then again from 2011 until her transition to this program. From 2005 to 2011, she was the resident service coordinator at Welles Country Village in Vernon.
The organization serves more than 12,000 individuals and families each year through regional offices located in Wethersfield, Watertown and Franklin. Corporate Offices are located in Bristol.
