OSHA faults Yale in lab accident

Federal safety regulators say an investigation into a Yale University machine-shop accident that killed a student found the equipment lacked required safeguards, The Associated Press reports.

The U.S. Occupational Safety and Health Administration wrote in a letter to Yale that the fast-spinning lathe lacked an emergency stop button that could shut off power. The Associated Press obtained a copy of the letter Tuesday through a Freedom of Information Act request.

Michele Dufault, a senior from Scituate, Mass., was killed April 12 when her hair was pulled into the lathe.

The OSHA letter said rules for using the equipment were not posted and said Yale should establish a two-person rule in which students don’t work alone.

OSHA did not fine Yale, saying it lacked jurisdiction.
