Community Health Center Association of Connecticut Inc. (CHCACT) will receive a nearly $5 million award as part of the Transforming Clinical Practices Initiative to improve care and patients’ access to information and cut costs, Health and Human Services Secretary Sylvia M. Burwell announced Tuesday.
The award is the first of four annual awards that will total up to $17.25 million.
The funding was among $685 million in national and regional awards to 39 groups to support changes in America’s healthcare system. The awards are part of an initiative advanced by the Affordable Care Act to strengthen the quality of patient care and spend healthcare dollars more wisely, HHS said.
