The Malta House of Care Inc. in Hartford is among 33 free and charitable clinics receiving grants from the CVS Health Foundation to support chronic disease management and prevention, improved care coordination and increased access to care programs.
Malta House, which operates a free mobile medical clinic through a volunteer staff of physicians, nurses and nonmedical volunteers, will receive $35,000 of the more than $1 million in grants given as part of a multiyear grant program with the National Association of Free & Charitable Clinics (NAFC).
The CVS foundation has donated nearly $5 million to NAFC over the past four years to increase access to quality care and support the management of chronic disease, the foundation said.
Support given to previous grant recipients is making a meaningful impact on their goals of improving care coordination, managing chronic conditions and increasing access to care, the foundation said.
