Hartford HealthCare (HHC) will provide on-site primary care and geriatric services at Seabury, a 50-plus community in Bloomfield, under an agreement announced Tuesday.
After-hours and weekend coverage will also be available virtually and in-person through the Hartford HealthCare Geriatric Medical Group and Hartford HealthCare-GoHealth Urgent Care.
“Our substantial network of local urgent care centers, online registration, Virtual Visit capabilities and integrated medical records will give the residents and members of Seabury access to great care they can trust, and the resulting peace of mind that comes with it,” said Dr Eric Walsh, medical director of Hartford HealthCare-GoHealth Urgent Care.
Hartford HealthCare at Home has also been added as a preferred provider for Medicare Home Care services for Seabury residents and Seabury At Home Members.
Dr. Somwya Kurtakoti, HHC chief of geriatrics, and Irsal Cahburg, will be the first team of providers working on the Seabury campus. Two additional providers will be deployed in February.
“It is important for older adults to have access to quality geriatric primary care” Kurtakoti said. “Our partnership with Seabury is an important step in expanding these services.”
Set on 66 acres in Bloomfield, Seabury operates as an interfaith “Active Life Plan Community” sponsored by Church Home of Hartford, Inc., an affiliate of the Episcopal Church in Connecticut.
