Hartford HealthCare announces new platform for patient engagement

Hartford HealthCare (HHC) will partner with Chicago’s Upfront Healthcare to improve its interactions with patients, the company announced Thursday.

Upfront’s products feature personalized content delivered over multiple communications channels with the goal of motivating patients to participate in preventative care, show up for appointments and comply with treatment.

“Upfront will play a critical role in helping us reinvent the patient experience,” said Dr. Barry Stein, vice president and chief clinical innovation officer at HHC. “We have to meet patients where they are and provide as smooth a journey as possible, digitally guiding them to the services that they need.”

HHC plans to use Upfront’s products to provide digital communication delivered via personalized landing pages embedded in text or email messages as well as interactive phone system technology.

Upfront’s main product is its Care Traffic Control platform, designed to direct patients to relevant care options within a health system by way of advanced analytics, personalized content and calls-to-action. Other products focus on care gaps, wellness visits and medication adherence, in addition to one geared toward HHC’s Medicare Advantage plan.
