Hartford getting money to improve coordination of health care and housing assistance for HIV patients

The city of Hartford Health & Human Services Department is among five public health departments and nonprofit groups in the U.S. selected to share up to $1.5 million in federal funding to improve the coordination of health care and housing assistance among providers who offer services to people living with HIV.

The funding, which will be spread over 2½ years, is provided through U.S. Department of Health and Human Services Secretary’s Minority AIDS Initiative, according to a news release from RAND Corp., which is providing support to the five sites and conducting the multi-site evaluation. RAND Health is an independent health policy research program, with a broad research portfolio that focuses on healthcare costs, quality and public health preparedness, among other topics.

The goal of the project is to improve health outcomes for persons living with HIV by enhancing the ability of providers to coordinate client information sharing and services across multiple funding programs, the release said. The project will focus on improving data systems, and coordinating housing and care services for clients across providers.
