CT’s Hughes Health, Hartford Hospital cardiac partners

Hughes Health and Rehabilitation Inc. and Hartford Hospital are collaborating in a program aimed at curbing unnecessary readmissions of patients with congestive heart failure.

Hughes Health’s 170-bed facility at 29 Highland St. in West Hartford has been participating for some time in the program with Hartford Hospital, said Hughes Vice President Mark Finkelstein.

The program involves, according to Finkelstein and Beth Lawlor, case management director of Hartford Hospital, constant monitoring of cardiac patients’ heart function, as well as proper drug dosing and rehabilitation, to avoid a relapse.

Similar programs to lower hospital readmissions are underway between long-term providers and hospitals throughout Connecticut.

“It’s an effort to contain costs and improve the quality of care patients receive,” Finkelstein said.
