Anthem Blue Cross and Blue Shield in Connecticut and Community Medical Group (CMG), an independent practice association of more than 950 physicians based in New Haven, said they have updated and expanded their value-based contract.
Full terms weren’t disclosed, but Anthem said CMG will gain access to additional data tools to help its doctors close care gaps, use best practices for disease managements and reach out to patients who are overdue for immunizations or prescription refills.
CMG is entering its fourth year in the Anthem program, called Enhanced Personal Health Care.
