CT Community Care

Mission: Connecticut Community Care Inc. (CCCI) identifies choices and provides services to help people of all ages, abilities and incomes to live at home.

Background: CCCI, incorporated in 1980, is the largest care management access agency for the Department of Social Services’ Connecticut Home Care Program For Elders and a similar program for adults with disabilities. Last year, CCCI helped more than12,000 elders and individuals with disabilities at risk for nursing home placement, to live at home by coordinating the in-home services they needed. More than 200 agencies offering homemaker, companion, visiting nurse, meal delivery, adult day care and other services, contract with CCCI to care for those residing in the north central, northwestern and eastern regions of the state. CCCI also assists individuals moving from skilled nursing facilities to community settings through the federally funded “Money Follows the Person” program.

Care Management Associates (CMA), CCCI’s private division, provides assessments, care management and in-home services for individuals who are not eligible for state aid on a fee-for-service basis. CMA’s personal assistants help individuals with appointment scheduling and transportation, meal preparation, light housekeeping, and more. CMA also provides assessments to individuals with Multiple Sclerosis through a partnership with the Connecticut chapter of the National MS Society.

CCCI uses funds donated to the “Independent Living Fund” to provide charitable care management, direct services or medical or personal equipment to its clients in need.
