Farmington-based health plan ConnectiCare said it has signed a contract with one of the largest medical groups in the state.
Under the “collaborative” arrangement between ConnectiCare and Yale Health-owned Northeast Medical Group, which includes more than 115 practices with 700 doctors and other providers, the two will use care management teams that aim to reduce costs and improve health outcomes.
The EmblemHealth-owned plan said it has similar arrangements with 13 practice groups around the state, including Hartford Healthcare’s Integrated Care Partners, St. Francis HealthCare Partners, ProHealth Physicians, and Grove Hill Medical Group.
It inked its first collaborative contracts for commercial-plan customers last year.
