Farmington health plan ConnectiCare said it has expanded its practice-collaboration agreement with the Connecticut State Medical Society Independent Practice Association.
The agreement now includes CSMS-IPA providers in five counties: New Haven, Middlesex, Fairfield, Windham and New London.
Financial terms were not disclosed.
ConnectiCare, a subisidiary of New York’s EmblemHealth, has similar deals with 11 other provider groups in the state.
It says the arrangements have resulted in increased utilization of primary care and fewer hospital admissions and emergency room visits.
The insurer launched its first collaborations in 2008 for Medicare beneficiaries. Last year, it expanded the arrangements to commercial members.
