Bloomfield health insurer Cigna on Wednesday announced it has launched a new collaborative care initiative with the Providence-Swedish Health Alliance in western Washington state.
Cigna launched its Collaborative Care initiative to help populations become healthier and reduce their medical costs, which are goals outlined in the Affordable Care Act and of accountable care organizations (ACO) like Providence-Swedish.
The deal reached Wednesday impacts 7,100 individuals covered by Cigna health plans who receive care from 1,500 ACOs.
The initiative will help close gaps in care such as missed health screenings or prescription refills, reinforce the appropriate use of emergency rooms, increase the number of preventive health visits and improve post-hospital patient care.
