Middletown-based Community Health Center Inc. said it has received a $1.2 million federal grant to expand its nurse care-coordination program across its Connecticut locations.
CHC said the goal of the program, which provides training and development to nurses, is to promote regular collaboration between nurses and other health providers to help them intervene at critical times for complex patients.
CHC will collaborate on the project with Quinnipiac University and Middlesex Hospital Homecare.
The funding is from the Health Resources and Services Administration, an agency of the U.S. Department of Health and Human Services.
