CareCentrix’s new cloud platform helps transition patients to home

Hartford-based CareCentrix, which connects patients at home to a national network of more than 8,000 credentialed provider locations, today announced the launch of a cloud-based platform to help patients remain at home and avoid unnecessary hospitalizations and readmissions.

The HomeBridge platform, built on Salesforce Health Cloud, was developed to provide a complete view of a patient’s data, track clinical events over time, and collaborate on care. It allows CareCentrix to connect patients and care teams to facilitate continuity of care as patients heal at home, according to a news release from the company.

Home is the underutilized setting of care that can help appropriately accelerate the transition from high cost fee-for-service models to value-based care, CareCentrix said.

“Patients deserve better care and coordination,” John Driscoll, CEO of CareCentrix, said in the release. “Too often healthcare providers play their position, but do not have the tools to work as a team to help patients transition home. With our HomeBridge platform, we align patients with the right providers and support those providers to work as a team to guide the path home and then continue to provide patients the support they need.”

With the real-time, continuous HomeBridge connection between patients, providers, centralized nurse teams, telehealth providers and community resources, the patient’s entire care team will be able to see a daily record of information like blood pressure and heart rate, and intervene when needed, the company said.
