Hartford-based CareCentrix said it has extended its contract with Massachusetts-based Fallon Health, a not-for-profit healthcare services organization and a client of CareCentrix subsidiary Sleep Management Solutions (SMS).
The three-year extension gives Fallon’s membership continued access to diagnostic testing and therapy adherence programs for sleep-related disorders, such as Obstructive Sleep Apnea (OSA), coordinated by SMS.
Since 2009, Fallon Health and SMS have helped more than 11,000 patients receive sleep-related diagnostic testing in their own home.
For those members with OSA, the SMS program assists each member to get started on the right sleep therapy and then supports patient progress by remotely monitoring the data from the patient’s positive airway pressure device to help ensure the therapy is working and to intervene when the patient is experiencing difficulty.
